We encourage you to review the resources here and hope we have been able to answer many of the questions you may be having, but as always, if you do have questions or concerns, please feel free to contact your family doctor at Davisville Family Practice.

The Government of Ontario recently unveiled a provincial-wide vaccine passport plan.  As of September 22, 2021 individuals will need to provide proof of being fully vaccinated (two doses plus 14 days) in order to enter certain public settings and facilities.  This movement is an important step in encouraging all eligible individuals to get vaccinated to keep everyone safe, allow business to remain open, and to protect the already strained hospital system.

To find out more on how to obtain proof of your vaccination please go to the following helpful link:

We have added below a link to our office policy on letters of exemption for the COVID-19 vaccine.

Thank you for allowing us to continue to be involved in your care.

Sincerely,

Davisville Family Practice
